   Were you named after someone?
   Do you know why you got that name? Any nickname as a kid?
   Any different name in your workplace?
   Do you like your name? What different name would you choose?
  What name would your parents use when you got in trouble?

When were you born?

Where were you born?  (city?  home? hospital?)

Where have you lived? Do you remember any of the addresses in case I could see it one day?

How did you get to elementary school?. middle and high school?

What foods didn’t you like as a kid?  Are there some you still don’t like?

  Are there some foods you hated  as a kid that you like now?

Did you have a favorite teacher and what made them your favorite?

What were your favorite playground games?

Did you belong to any school clubs?

Did you win any school awards?

What after school activities did you do?

Did you have music lessons?  What instrument(s)?  How long did you take    

   lessons? How much time did you practice? What did learning an instrument do for you?

Do you remember some family trips?  Tell me about a couple of them.
What were your parents like?  What did you call them?

Did you get enough love, appreciation, encouragement and hugs from your parents or others?
  If not, what did you think the reason was? What do you think about it now?

What were the rules in the house for you? Chores? responsibilities?
